We are in receipt of your request for Certificate Extension due to a pending location change. Please review the attached “Notice on Extension”. In order to be eligible for certificate extension ASME requires that both application and certificate/review deposit fees be received no later than 6 months prior to your Certificate expiration [May 12, 2013]. Your certificate is currently under renewal and in order to meet the certificate extension option outlined on page 1 of the attached you will need to have your application/fees received by ASME no later than November 30, 2012. If an invoice is necessary for payment please submit your request for one to [email protected] .

If however, you cannot meet the November 30, 2012 deadline then your only option would be to have your AIA/OneBeacon America perform a shop audit as outlined on page 2 of the attached notice. You will still need to get your application and fees plus the extension fee of $2,000 submitted to ASME by February 10, 2013.

In regards to the location change we can process it at the time of your review or after. Attached are the necessary renewal forms that must be completed, signed, dated and submitted with payment.
